Athlete/Activity Forms

The following forms are required yearly for all 7-12th grade athletes and some student activity events. 

• Athletic/Activity Medical Treatment Consent Form - PAPER COPY REQUIRED
• Athletic Physical Examination - PAPER COPY REQUIRED
• Concussion Fact Sheet - TO BE SIGNED DIGITALLY WITHIN JMC

The following is now required for all K-12 students (not just athletes) and is signed digitally within JMC during registration. 

• Parental Insurance Waiver - TO BE SIGNED DIGITALLY WITHIN JMC

For passes to home athletic events, follow the link below to order an Activity Pass.
