The following forms are required yearly for all 7-12th grade athletes and some student activity events.
- Athletic/Activity Medical Treatment Consent Form - PAPER COPY REQUIRED
- Athletic Physical Examination - PAPER COPY REQUIRED
- Concussion Fact Sheet - TO BE SIGNED DIGITALLY WITHIN JMC
The following is now required for all K-12 students (not just athletes) and is signed digitally within JMC during registration.
- Parental Insurance Waiver - TO BE SIGNED DIGITALLY WITHIN JMC
