Vaccine Clinic Coming to THS on May 20th

DATE: Thursday, May 20 (2nd dose will be June 10th at the school)
TIME: 9:00 am-12:00 pm
LOCATION: Treynor High School East Gym
ENTRANCE: Enter through the High School Entrance
INFORMATION:
- Must be 12 years of age or older on or before 5/20.
- Pre-registration is not required. You can attend the clinic anytime between 9:00-12:00.
- Complete the consent form and bring it to the clinic.
- Students under the age of 18 must have the consent form signed by a parent, and parents do not need to be present.
PARTICIPATION: This is a voluntary vaccine clinic, and it is open to any student, school staff, or community member age 12 and up. If you have questions, please contact Mrs. Cory Young at 712-487-3181 ext 7.
